2009 School of Public Health Mentor Program Student Application


Steps:
The School of Public Health Mentor Program provides an opportunity for students to develop professional skills outside of the classroom and begin to build a professional network. Students and mentors are matched based on mutual interests and mentors' expertise. The mentor/student relationship is informal with monthly contact by phone, email or in-person.

Students participating in this program are not only introducing themselves to their chosen professional field, but also representing the School of Public Health to its alumni and other professionals in the public health community. As a result, we have high expectations for students who choose to participate in this program. It is important for you to read the expectations below carefully to determine if you will be able to make the commitments needed to make the mentoring relationship a success.

Expectations of Students

1. Attend the Kick-Off event to meet your mentor.

2. Initiate regular contact with your mentor via phone, email or meetings.

3. When you meet your mentor, discuss your goals for the program and both of your schedules. Because both graduate students and mentors have busy schedules, plan future meetings each time you meet.

4. Meet your mentor at least once a month or as determined by the goal you set together.

5. Act in a professional manner at all times. Return phone calls and email correspondence in a timely fashion.

6. Contact your mentor and the Mentor Program (612-626-5536) immediately if circumstances require you to discontinue your participation.

7. Please notify the Mentor Program if your mentor is not fulfilling his or her responsibilities.

8. Although participation in the Mentor Program may lead to a possible internship or field experience, this should not be an expectation of the program.

By completing this form, I am indicating my willingness to participate as a student in the Public Health Mentor Program for the 2009-10 academic year (through May 2010). I have read the above expectations and understand my responsibilities. I will receive information about my mentor in October and will initiate contact with that person prior to the Mentor Kick-Off event. Finally, I understand that as a participant in the Public Health Mentor Program, I am also a representative of the School of Public Health and the University of Minnesota.
